By submitting this form, I understand that I/ my team will need to register at 8:00 am the morning the event at the Fall Creek Golf Club in Pendleton. An application will need to be filled out and signed by each player that includes a waiver to be eligible to play. Players must be 16 years old or older, but anyone under the age of 18 will HAVE to have a parent sign and complete their waiver form.
